Public information request about AI systems

Prepare a letter asking a public institution whether it uses AI systems or algorithms in matters concerning people, and on what terms.

The data you enter in the form does not leave your browser. Nothing is sent to a server or stored. When you close the tab the data is gone. You send the letter yourself, in your own way.

This is a template, not legal advice. Check the letter before you send it. Everything concerning legal provisions is marked and waits for a lawyer.

Legal basis

The letter has a field for the legal basis. We do not put the name of an act or article numbers in it until a lawyer has checked them.

[to be completed by a lawyer] The name of the act, article numbers, how to cite the basis in the request, how and in what form to submit it, the related time limits, and whether a request to a Polish institution should be written in Polish.
